Responsibilities And Key Accountabilities

Permit Issuers are trained and approved persons responsible for approving Work Permits. Roles/Responsibilities include, but are not limited to:

Informing the Permit Holder of the work to be done, the exact location of the tools and plant /equipment to be used in the work, the hazards that may be present and all the precautions prescribed on the Work Permit that must be taken during the work and ensuring the Permit Holder has transmitted those instructions to his staff and Contractors

Identifying and addressing any conflicts between the proposed work and other activities in the area, and if necessary cross-referencing the relevant Work Permits in the location

Confirming that all hazards associated with the work have been identified during the work preparation process and all relevant parties have provided their input and such parties agree with the precautions to be taken during the work activities, e.g. through supporting

Ensuring that all legal requirements relevant to the work have been fulfilled prior to issue of the Work Permit and continue to be satisfied during the carrying out of the work subject to the Work Permit. Ensuring that all necessary supporting Certificates, Job Hazard Analysis and drawings are attached to the Work Permit

Confirming that all operational preparations (including process clearance and isolations) have been completed.

Confirming, prior to commencement of the work, that the conditions in respect of the worksite have been met and therefore it is safe for the work to proceed.

Describing in the Work Permit what precautions need to be taken during the work, including use of safety equipment and Personal Protective Equipment (PPE).

Notifying all people who may be affected by the work covered by the PTW System that the work Permit has been granted.

Signing and issuing the Work Permit to the Permit Holder.

Displaying the status of the work at all times (e.g. on a display board) and communicating the status to the oncoming shift at shift handover.

Signing off and cancelling the Work Permit when the work is complete, abandoned or suspended.
